Created Heady Glass Bong (#9645) with custom messages and descriptions.

Added effect::merge_with on #9479 - returns LIST of new waifs for merge/cancel/interact.

Fixed $effects_pc:add_effect waif copy bug - uses merge_with + listset.

Fixed test_add_effect to save/restore player effects state.

Updated design spec (note #9473) with v2 smoking mechanics and future plans.
